Referring a patient
to Sleep Management Centers: Physicians from any
specialty can refer patients directly to the sleep center
by completing the SMC
Referral Form and faxing it to 239-210-0048.
This should also include the patient’s history, physical
information and insurance information. We will contact the
patient to schedule the sleep study for the next available
appointment or at the patient’s convenience.
Physicians can also access a number of patient questionnaires
by using the convenient login field located in the upper
right hand portion of the home page. Please call SMC direct
at 239-334-8144 for your access code. Patients
will be instructed to bring these questionnaires with them
on the night of their sleep study. Patients should also
be given the SMC
Study Instruction Packet prior to leaving their physicians
office.
Once a patient is referred to us for a sleep study, we will
contact the patient to provide all the information they
will need to have a comfortable stay in the center. Following
the sleep study, the recording is scored by a Registered
Polysomnographic Technologist and interpreted by one of
our Board Certified Sleep Specialists.
Physicians can also refer patients directly to our Medical
Director, Dr. Alsbrook, for a thorough interview and evaluation
of their sleep-related complaints. Following the interview
and office visit, the patient will be scheduled for a sleep
study if required. Direct referral to Dr. Alsbrook by calling
239-334-8144.
When patients are referred directly to the sleep center,
results are sent back to the referring physician following
interpretation. Depending on the results of the first night’s
study, our Medical Director may recommend that the patient
return for a follow-up sleep study, perhaps for a CPAP titration.
Once the patient has completed all the testing and the results
are interpreted, a complete report will be sent back to
the referring physician. Reports include a summary of the
study and the diagnosis and treatment recommendations that
can easily be followed by the referring physician.
