Sleep Management Centers LLC (SMC) has developed a unique
business model that offers partnerships with physician groups
that are interested in expanding services provided to their patients.

Although specializing in the “Cardio-Sleep Link” all practicing
physicians are invited to consider this partnership.

SMC intends to establish and manage a consortium of
Sleep Disorder Centers to further advance the diagnosis
and treatment of sleep disorders within the state of Florida.

The Proposal


•	SMC will build, staff and manage a 6 to 8 bed Sleep Disorders Center in partnership with your practice
•	Your practice will have a percentage ownership in your individual center
•	There is a minimal partnership investment from your practice
•	One member of your practice will serve as an officer of the newly formed corporation’s board
•	This member will also serve as Medical Director of this center
•	SMC will work with your staff to screen your patients for referral
•	SMC will help coordinate follow-up and support for the patients

Why Partner with Sleep Management Centers?

Unlike large multi-site sleep organizations, SMC plans to only set up Sleep Disorder Centers in the state
of Florida. This ensures a smoother transition when entering into a partnership, constructing the Sleep
Center and the day to day operations since the SMC management team is experienced in state permitting,
contracting, licensing and legal issues. This management team also has 75 years experience in sleep
disorders medicine and polysomnography, which includes Sleep Center design, staffing, diagnostic testing,
study interpretation, billing and collections. Management experience in all business aspects at this level will
ensure operational success and quality patient care.

The field of sleep medicine is projected to grow considerably over the next 5 years as public awareness
grows and research continues to strengthen the link between Cardiovascular disease and Sleep Disordered
Breathing. This will become more evident in Florida as an aging population with a higher prevalence of
